Volunteer Forms

All volunteers must have this reference form completed and sent to Volunteer Coordinator Mark Holder as soon as possible after registering as a volunteer:

All volunteers who will be 18 and over when they serve Summer 2012 must complete a Background Check form and send to Volunteer Coordinator Mark Holder as soon as possible after registering as a volunteer. Forms should be mailed to the address on the form or faxed to the fax number on the form. There is a $20 charge for all background checks, which should be paid in addition to your registration fee.

The following form should be completed and BROUGHT WITH YOU TO CAMP. DO NOT MAIL THIS FORM. Bring this form, with your medications, to camp. This form is required even if you do not have medications to be taken at camp. If you are under 18, this form must be signed by a parent or guardian.
